      Health app development in Spain: technical requirements, regulation and user experience

      Digital health in Spain is moving fast, pushed by the pandemic, mass smartphone adoption and European regulatory change. But building a successful health app is significantly harder than building a consumer one: health data is special-category under GDPR and needs technical measures well beyond standard encryption, SaMD regulation may apply depending on intended use, and the user experience has to work for clinicians and patients whose contexts could not be more different.

      Health data is the most protected category under GDPR. Article 9 requires additional technical measures: separate explicit consent, role-based clinical access, auditable access logs, specific breach-notification protocols and mandatory impact assessments. Any app that collects or processes health data — including wellness data such as heart rate, weight or menstrual cycle — has to meet them, and that is decided at architecture time, not bolted on before launch.

      Integration with electronic health records is one of the hardest technical areas in the sector. HL7 FHIR R4 is becoming the European interoperability standard, but many Spanish hospital systems still run older HL7 versions or proprietary systems with no documented API. Our health work includes integrations with several Spanish EHR systems, interoperability middleware and format transformers to normalise that heterogeneity.

      User experience in health apps has constraints no other sector has. Clinicians use the app wearing gloves, in low light, with maybe thirty seconds per interaction. Older patients need large type, high contrast and simplified flows. Carers use the app while attending to someone else. Designing for those contexts requires sector-specific UX research — observing users in real clinical settings, not just interviewing them.
